That I am competent to make this affidavit.
That I am holding two Director Identification Numbers (DIN) as per the details given hereunder

That I hereby apply for cancellation of the DIN No. : ….. on the following grounds :-
i) That the DIN has been applied and allotted by an honest mistake from my part.
ii) That I have no malafide intention to hold multiple DIN.
iii) That the DIN No. ….. has been inoperative from the date of allotment.
iv) That the said DIN has not been used before Ministry of Corporate Affairs and/or any other authority
v) That I am not holding Directorship in any company with the said DIN.
I solemnly state that the contents of this affidavit are true to the best of my knowledge and belief and that it conceals nothing and that no part of it is false.
That para 1,2,and 3 of this affidavit is true and correct and para 4 is my affirmation.
