14 August 2012 You need to file form 61 to the ROC along with an application on the letter head of the company in this regard and an affidavit from the directors of the company on stamp paper of Rs. 100

20 August 2012 Write the application stating that due to certain reasons company could not file AR & BS and requesting the ROC to change the status of company on master data from "Dormant" to "Active". Print it on the Letterhead of the company and ask the director to sign it and attach it with form - 61.

100 Rs. Stamp paper & affidavit is not compulsory.
