Name bhaskar b
Fathers name balayya
Which of the following affidavit format is correct if my ID proof contains single name i.e.Bhaskar B ?
1. Iam BHASKAR B and my name bhaskar b appearing.............
or
2. Iam BHASKAR BALAYYA and my name baskar b appearing........
( how it is to be signed (a) bhaskar b or (b) bhaskar balayya )
i want to clarify that you have to write you surname in the DIN Form...firstly you have to write the name in the Din form for which you have the proof to validate...and secondly it seems that the 'b' along with your first name is used only as alphabet...so u have to attach an affidavit notarized from a notary that alpabet "b" stands for .....and send the same with the Din application....
