25 April 2011 when we talk about address proof it should be for permanent address or present address or either of them can be accepted

25 April 2011 Clause 12 of DIN-1 is for permanent address and clause 13 is for present address. If the above addresses are not same proof of both should be attached with DIN-1 because the words used in instructions is in plural viz.,"address proofs".
