Deemand for balance tax

i have deposit income tax's demand before last year against income tax demand letter but now i received same letter form income tax department for same demand and now i want to give reply to income tax office with application so i request u plz send me application letter's format this matter

09 October 2011 Income tax department always send a response sheet with demand note or any query, always keep a copy of letter send to department. You can also write a simple letter along with copy of challan of tax deposited by you.
