Declartaion under 11 a b - excise

I have paid interest on supplimentary invoices made during the year at the time internal audit of excise department. Officers told me that give me a declartion under section 11 a b so i will not issue a showcause notice to you. so please will you give me that declaration format.

make statement of your payments made with challan xerox and handover to audit stating that on your own will and with help of audit team, you have paid the interest calculated,
make copy to range superintendent and deputy comm of your division.
11AB is the section, that if you dont pay and submit then they will issue S/C
