Correction of registration date of articleship

i have made a terrible mistake by entering the wrong month in the registration date i.e. 15/12/12 instead of 15/10/12 in the form 103 due to which i am loosing my attempt in november 2014 and it came to my knowledge when the confirmation letter came from the institute...so please tell me how to rectify it..!!!

You should quickly contact respective regional office of Institute with Letter of your C.A. and other relevant documents in support. I emphasis for personal visit at office . In past I found that Institute is very much co-operative in the matter.

Correct it by sending letter to ICAI and by sending fresh form 103
