Correction of challan assessment year

06 October 2014 I had submitted a challan online but the assessment year mentioned was wrong 2011-12 instead of 2012-13. As per the process I need to submit a physical application to AO. My AO details are:
Area code: NWR
AO Type: W
Range code: 62
AO number: 1
Jurisdiction: Ward-1, KUK
Building Name: Kurukshetra, Haryana

From where do I get the address to which the application is to be sent? I am not able to find it over any site as of now
