Conversion of full time cop into part time cop

I have two questions -
1. What is the procedure to convert Full time COP into Part time COP.
2. One of my friend is holding COP from last 3 years but now he is about to join a job. In case he surrender the full time COP and restore it back again after a gap of 2 years in job, then will the previous 3 years of COP be counted for FCA when he completes 2 years of COP again after restoration
Please help as it is very urgent
The earlier 3 years experience as full time is counted.

A simple formal letter intimating ICAI for conversion of Full time to Part time is sufficient or a separate form is prescribed for the same
