After forming Audit Committee is there any requirement to inform this to ROC. Is there any specified form.

Not required. When you are publishing the annual accounts along with the directors report you have to give the details as per sebi guidelines about the committees, the directors incharge of the committee, number of meetings held, their attendence and any adverse remarks made by the committee and the replies made by the Board of directors has to be disclosed to the shareholders.
