Compeletation of articleship

I started my training as on 26.06.2013. I got tarmination as on 08.03.2014. I started my training another firm as on 23.03.2014. According to my articleship entrance date my training is completing on 25.06.2016. There is 14 days gap between my termination and joining date consider as extend days of training ???
If It is then what is the process to complete it and which form i have to fill after its completion ???

you can fill the form with new employer with the extra period of gap. or may do after completion of article period.
