Certificate of practice

Dear Sirs,
I have applied for Certificate of Practice and I got the letters from the institute granting the same. I am planing to start it as a proprietorship in my own name initially. The letters I received from institute includes FORM 117 (http://resource.cdn.icai.org/8589form%20117_final.pdf) . Do I need to fill it up and send, or whether it is needed only if I join a partnership firm or if it started with a trade name? Is it required to be send for starting a proprietorship in my own name?
Another annexure is a small card (annexure C) with 4 columns for specimen signatures. Do I need to send it back with all the columns signed?

No need of 117 if practicing in own name. Pl sign in all the columns and send back to ICAI.
