Dear Madam/Sir

What is this SPECIMEN SIGNATURE CARD required to be affixed with the Membership Form i.e. Form A ?
From where I can obtain this specimen Signature card?

As different Amount is stated for different Purpose so can I make SINGLE DD for all fees relating to Associate membership?
And in Column 9 of the 'List of documents required to be affixed with Form A' from whom i should take attestation ? Whether self attestation will suffice?

03 April 2013 Yes, you can make a single DD for all the fees.But the fees will vary depends upon the period of registration.If u register for ACS membership between oct to mar then your mem. fees is Rs.562.50/ +Entrance Fee Rs. 1500+
Necktie/Lady broche Rs. 50/-total amounting to Rs.2112.50/-. If after mar then your mem fees will only change rest of the fees will be the same and you have to pay Rs.2675/-.

You have to produce (i) Two fitness certificates from two members having at least three years standing as members of the Institute.
