I have decided to terminate my articleship stating reason that I want to join employment but they reject it and now asking to change the reason to discontinuation. Can you please help me with the affidavit that I have to upload.

06 April 2023 Respected Sir, When I filed Form 109 in that I have selected the option student joining employment but that got rejected with a remark to choose the option for discontinuation and upload the affidavit. I am unable to find the format of affidavit. If you can povide me that would be of great help. Thank you!
