20 May 2011 everthing is mentioned in Form 112 itself...kindly read the form 112 carefully....

normally, it is to be sent within 30 days of joining that course....

procedure is to send the form 112 duly filled along with a condonation letter for delay and condonation fees is as under-

delay upto 30 days - 100/-
delay 31 to 180 days - 300/-
beyond 180 days - 1000/-

yeah it is to be signed by your principle only and cant be signed by any other CA..
