what is the procedure to cancel C form and reissue it.

You need to write a letter to your VAT officer regarding this and get the return for the year reopened. Then revise the return and take revised Form C.

I mean to say- VAT officer means your Commercial Taxes Officer.
Only revising the Sales Tax return can help you in revising the C Form. For that, you need to write a letter to your Sales Tax Officer to enable you revise the return. Then revise the return and generate a new C Form.
