Bank signatory authorized form

I have a current account with SBI for my partnership firm , I want to give authorized signatory to my accounts manager. What form i should use?? please send the form for the same!!!

Specimen signature form be used.

Bank Person telling that there is form "mandate" but there are not yet give that form to operate the account.
