I have appeared in MAY-15 but i didn't pass. I was unable to appear in Nov 15 exams. Will Nov 15 be counted as attempt even if i haven't fill the exam form? Please clarify.

As per my understanding if you have not filled the exam form, this nov 2015 attempt will not be counted for the purpose of number of attempts.
