I completed my 1year 7months of my Articleship period, now unfortunately i met with an accident recently which affedted my lower backbone, which requires a treatment under specified conditions for 12 to 14 weeks and i cannot attend my articleship training during this period, so i want to take a articleship termination on the basis of "Medical grounds requiring discontinuance of articles for a minimum period of three months". So I request you to please guide me regarding the procedure to be followed in a detailed way, however i studied the 'Regulation 56(1)' relating to "Transfer/Termination of Articleship" which says that "A prior consent of the Institute before getting Form '109' signed by the principal", even then i have some queries as mentioned below My main concerns are
1. What all documents to be submitted while making request letter for termination 2. When can i get the reply from Institute accepting my request for termination 3. When should i file Form: 109 4. Should i file Form: 109 in triplicate 5. Which will be my last day of articleship to be specified in Form: 109, means the date i sent a request letter for consent of institute or the date on which the relpy has come from institute accepting the request.

1. you may submit medical certificates evidencing your the gravity of your injuries and time required to recover
2. ICAI would take sometime to give the approval.
3. You need to file form 109 after receiving approval from ICAI.
4. you need to submit one copy to ICAI. You may photocopy two sets - one for your CA, one for you. You may get one of these copies stamped by ICAI as acknowledgment.
5. keep the date on which you sent the letter for approval to ICAI as the last date.
