Articleship termination

I had dispatched my articleship completion certificate on 19 feb 2014 with late fees but till now I have not received any notice or letter from ICAI kanpur. I have to fill my may 14 exam form. Then what should I do as the last date of exam form is 28 feb and I had already completed my articleship but not received any letter from institute.

Send the form. Enclose a copy of Form 109. Also enclose a letter mentioning the fact that articleship has been completed however, letter has not been received from the Institute.
