My articleship completed on 28.10.2011. Please advise as to which form and other documents are required to be submitted to institute in this regard. Is there any time period for the submission of documents?

1. form no 108 required to be submitted with signatures of emloyer.
2. DD of Rs. 100/- in favour of secretary,ICAI...because delay in submit form 108 exceeds 30 days
