21 July 2012 First & foremost, you need to submit Form 119 to your principal (employer). Form 119 is a Requisition Form for issue of service certificate. Based on this, your employer will give you Form 108 which is a Certificate of Services under Articles. You need to send Form 108 signed by your employer to the institute / council within 30 days of completion of your articles. If you send after 30 days then you need to pay Rs. 100 as late fees. Delay beyond 30 days upto 180 days requires payment of Rs. 300 as late fees. Late submission fees slabs are mentioned on Form 108.
Download form 119 from ICAI. Form 108 you should get from council or offices of ICAI.
In short send original signed form 108 (signed by employer), acknowledged copy of form 119 & a covering letter to ICAI within 30 days of completion of articles.
