Appointment of director as per companies act 2013

I have to appoint a director in Pvt Ltd Company.The director i have to appoint was Promoter of company but due to some reason he quit the company and give resignation.But now he want to again become director, Suggest me whether i appoint him Promoter, Professional or Independent Director, Also send me the format of Appointment Letter as per requirement of DIR-12.

Simply appoint him as additional director under section 161 and file form Dir 12 within 30 days of Passing board resolution.
For sample appointment letter pls search in share file section. I have uploaded the same in share file section. As per my opinion u may appoint him as promoter director.
