17 January 2015 In a Private Company , I want to appoint one of Director as officer who is in default , pl. know me which which form I should file with ROC under the Companies Act, 2013

19 January 2015 I guess no form is required. Only consent of appointee director & Specification (Resolution) is sufficient. Though, you may prepare consents in form of affidavit etc.
As Sec. 2 (60) (iii) does not provide for reporting such arrangement to any authority, no form is required.
