1. Board resolution authorizing the authorized signatory. In case of proprietor power of attorney required.
2. Declaration in Annexure-III/IV (as applicable)
3. Form ST-1 (on-line application)
4. Acknowledgement of on-line application
5. Address proof and ID proof of authorized signatory.
6. Copy of OLD ST-2
Note: all documents must be signed & stamped
