22 January 2014 Respected Sir,

Please give a format of Affidavit on the below mentioned subject matter.

"That a company was incorporated on 14th March 2013 but has not commenced business till 31st March 2013."

This has to be submitted to the Professional Tax Department for Requesting them not to levy professional tax for the financial year 2012-13.

Affidavit of Mr. Ramesh kumar veram, aged about 37 years S/o Mr. R.L. R/o _________________.
I, the above named deponent solemnly affirm and state as under:
1. That the deponent is the partner of the firm M/s _____________, situated at _________________, and hence is fully conversant of the facts deposed below.
2. That in the firm there are three partners including the deponent.
3. That the firm formation on 14.03.2013 but due to some technical problem could not started the business till 31.03.2013 .
4. we undertake/ declare That our firm have not closed the business till date .
5. That the deponent is therefore, partner of the firm with effect from 14.03.2013.
