Admit card without photo and signature

i have taken print out of my admit card for CA Final November 2015 exam..it is not containing my photo and signature.. what should i do now..instructions states that i have to get my signature and photo attested on a letter head of a CA..can anyone provide me content of attestation which i should write on letter head..plz help me out

u need to write a simple letter nothing else.. that u r working under him and he knows u and so on... along with that take one photo identity card in exam hall
