Income Only from Insurance Commission-194D, income below taxable (Rs.165000/-) and books is not maintain, which ITR form applicable for this?

ITR-4, Because of section 44AD clearly indicating Section 44AD doesn't apply for Commission agents and ITR-4S only applicable for the assessees who offering their income u/s 44AD.
So you doesn't comes under section 44AD, so must and should be fill ITR-4.

I will E-file the ITR 4 for Income from Insurance Commission -194D.
I have fill up the income details in Part-A- P&L , Item No.51, a. Gross receipt Rs.165000/- , b.Gross Profit Rs.165000/- & TDS Certificate details in Schedule TDS2 - 4 Qtr.
This ITR 4 Return is correct or not?

Yes, it is correct. Assuming you by mistake posted Rs.16500/- for gross receipts instead of Rs. 165000/-.
