UDIN update failed because of error in form code

i tried to upload Bulk UDIN .csv file. And mention form code F3CB-3CD for form 3CB-3CD. But after processing it shows format of form code should be valid. Please let us know the valid form code for form 3CB-3CD. And also let us know how to resolve this issue.
