Trf of ca articleship after completion of 3 year

My articleship is ending on 19/08/2014.There is extension of 5 months.I want to serve this period under other CA firm. What are the procedures & forms to be used.
Please reply ASAP.

you have to take prior approval from icai for transfer ..
