Transfer due to family shifting

Dear Sir,
I have completed 1 year and 10 month of Articleship. Due to some personal reasons my family deciding to shifting to another city. Is I take transfer to another city? If yes then Which documents I have to submit to institute?

Article can take transfer after one year of his artcileship if the new principal is 50 km away from the old principle ie he shifts to another the city. Form 109 has to be filled by article
