Termination of articleship

I have joined articleship on 12-12-12.now i have completed 2 years of articleship.now i want to take termination on medical grounds.plz let me know when should i submit the proof.now or after three months?when should i get the sign from my employer?when can i re-join in articleship under another principal?

17 December 2014 what medical gounds ur looking for..pls mention and u need to first on a plain paper mention the reason with a certficate from govt hospital doctor stating ur case on its letter head with all the other proof attached.

send this to icai with ur ca signing such letter and then when icai permits u , u can file 109 with icai.
