Surrender of pan card

I am running a proprietory business in the name of "ANNDAVAR POLYMERS". I have own PAN No(CIXPS1522E) in my own name. But Mistakenly i have applied PAN for " ANNDAVAR POLYMERS"by attaching SSI certificate as address proof, but the income tax dept alloted PAN to the company(ANNDAVAR POLYMERS) as a private limited company(PAN # AAICA1680P). So. pl. tell me how to surrender or cancel the PAN No in the name of "ANNDAVA POLYMERS"

IN THIS REGARD YOU ARE ADVISED TO FILLUP THE FORM FOR CORRECTION IF YOU WANT TO IN YOUR NAME BECAUSE YOU ARE THE PROPRIETOR OF"ANNDAVAR POLYMERS"
