Is their any provision for submission of Statement of affairs along with return of income in case of individual...

NO its no provision for submission of statement of affaires with return .
if individual have business income and his/her income exceeds rs. 120000/- in a year then book of account should be maintain and fill balance sheet also. or in audit case it also compulsory.
