Status holder certificate

Dear sir,
we are manufacture exporter and ISO certificate holder, we applied to dgft for status holder certificate. but they raised a quarry :
that our ISO certificate agencies not mentioned in that list of appendix 5 of DGFT.
and
they asking consolidated foreign currency statement from bank. what is that how to prepare. any format is available please provide.
if the above two are not possible to produce means what is the other remedy.
