21 February 2011 Dear Sir,

I submitted amended St-1 for change of address in September 2010 with photocopy of companies address proof like telephone bills etc. but i submitted amended st-1 which was filled-up online. now i have received mail from department to submitted amended form St-1 with relevant documents by 25/02/2010 otherwise it stands cancelled please help me what should i do

21 February 2011 Kindly give the correct information ...you had amended ST-1 in September 01 and Mail for Sercvie Tax Department to appear along with the documents dated 25/02/10..

if this date is 25/02/2011 then immediately submit amended ST-1 along with ST-2 issued earlier and Address Proof .....to the service Department....they will issue new ST-2 form....after verification of these documents

22 February 2011 Sir, will ST-2 be submitted original or copy of that and which type of document should be submitted regarding address proof of company (proprietorship firm)

22 February 2011 you have to submit Original ST-2, and service tax department will issue amended copy either new ST-2 or amended on the same ST-2 signed by the officer in-charges after verification of Address documents...
For proprietorship you have to submit the Address proof of the proprietor and Address proof of Place of Business...
