which form for public charitable trust have to fill up for short deduction of TDS on Rent & interest on saving account and fixed deposit.

You have to apply to concerned Income Tax officer of your jurisdictional TDS Ward/ range under Sec 197. Application should be accompanied with last year B/S, ITR & projected financial statement for the relevant year, for which lower deduction is seeking.
After duly verification, you will be issued certificate for lower deduction of TDS.
