Is it necessary to give share application form where shares are allotted as a reult of business takeover. Please suggest. Also for subcription to MOA is it necessary to give share application form?

In both of the cases you do not require to give share application as in takeover you get shares of other company as per basis decided by board & in case of subsriber to MOA mean they have sign MOA & shares are allotted on that basis, so do not rquire any share application.
