YOU MAY IMMDTLY. FILE REVISED RERURN OF ALLOTMENT TO ROC AND STATE IN IN ANY APPROPRIATE PLACE /COLUMN OF THIS REVISED RERURN THAT THIS IS REVISED RETURN AND IT IS IN FILED IN CANCELLATION OF ORIGINAL RETURN FILED ON DATE----
OR IN THE ATTACHMENTS TO THE REVISED RETURN ATTCH A LETTER TO THIS EFFECT.
ALSO ENSURE TO CALL BACK THE SHARE CERTIFICATES IF ANY ALREADY ISSUED (IN PURSUANCE OF THE ALLOTMENT ) TO RECTIFY ANY MISTAKES AND THEN REISSUE SAME.
