Regarding to form 112

i have registered to ca articles on September of 2012 by then i was pursuing my b.com but not regular ( correspondence). I didn't file form 112 then. So please help me regarding to filling of form 112, procedure and details to be enclosed clearly.

file form 112 along with specified fine.no other document etc is required for correspondence course.

in this case it is advisable to file form 112 alongwith condonation fees and condonation letter stating the reason for delay in filling form 112.
