SIR,
I am presently persuing ca and articleship before set-2008.so i have not applied for form-112.
but now i want to pursue cs also.so will i required to file form-112 with the institute.pls guide me for the same.

yes, you should file form no-112 after taking admission in cs(ex prog)because ur training is not completed yet.down load the form using following link and read the instructions.
