what are the documents required to apply for PAN of Minor ? & who has to do the sign on application form ?

If the applicant is a minor (i.e. below 18 years of age at the time of application), any of the documents as per the lists specified in instructions of PAN form in respect of any of the parents/ guardian of
such minor shall be deemed to be the proof of identity and address of the applicant.

In case of minor, if you have passport of minor, it will be a great proof for identity as well as for address.
Moreover, guardian can sign on behalf of minor.
