17 November 2013 if you already have membership no, you need to apply for COP in Form 6 along with a cheque of Rs 2000

In case you are yet to apply for membership, please fill form 2 alongwith Rs 4800 as fees (Rs 2000 for membership registration, Rs 800 for annual fees and Rs 2000 for COP). Please check for fees with ICAI in case they have recently done any upward revision.

Documents to be attached with Form 2:

1. articleship completion letter

2. marksheet for CA finals

3. Birth certificate/class X certificate

4. Marksheet of all education qualifications

5. GMCS certificate

6. I-card Form
