Dear Sir,
Can I see the physical form for Nov 2013 IPCC exam on Insitute site or elsewhere? Please suggest me because for claiming the exemption got in Nov 2011 not any place in online form for claiming such exemption because in online form exemption month & year already printed.

go to your regional office and ask them for a form.
Though there must a column for the same in online form as well,because there are numerous students who apply online and claim exemption

further your exemption was valid till november 2012

if u got exemption in nov 2011 and u couldnt clear the group then ur exemption is lapsed as for only 3 next attempts the exemption is allowed.
for example if u got in grp-1 in nov 2011 exemption shall be valid till next 3 attempts from nov 2011 ie may 12, nov 12 and may 13.
this is irrespective whether u appeared or not for these 3 attempts so even in physical form u will not be allowed any exemption u got in nov 2011.

But sir I am taking the conversion on the basis of direct entry basis & as per announcement of ICAI dated 24/07/2013 I am eligible for such exemption. Therefore in that case what shall I do?

if u got exemption in nov 2011 and u now converting to direct entry scheme still u wont get any exemption as per rules of examination by icai in this regard.

But sir as per rules of examination exemption will be carried forward for next 3 exam i.e. in my case May 2012,November 2012 & May 2013 but due to taken entry on the basis of direct entry I was not eligible for May 2013 exam & my last attempt for claiming the exemption is November 2013. Is it correct or not?

there is no specific criteria notified by institute in the event of conversion or direct entry basis.Accordingly your exemption was valied till may 13only.Contact examination department once.
