14 January 2011 For ID and address proof, use any of the following:
1. Bank Ac First Page (you can open one if you don't have, it is easy)
2. Ration card of parent
3. Birth Certificate
4. School admission certificate (i doubt but pl check up. But this may be used for opening bank ac. and then with bank ac copy apply for PAN
