I NEED A FORMAT OF AFFIDAVIT BY KARTA OF HUF DECLARING FATHER'S NAME AND NAME AND ADDRESSES OF MEMBERS OF HUF AS ON THE DATE OF PAN APPLICATION FOR APPLYING PAN IN THE NAME OF HUF

There is no need for such declaration and such affidavit for the names of members. It is not asked in the Form 49A.

It is so required as per the instructions printed in Form 49A. The form may be downloaded form the website of ITD. However there is one more thing in the English version it talks of an affidavit while in Hindi version it talks of Declaration (Ghoshana Patra)
