fill 49A and send with requisite fees, documentsand photo, a new pan card will come.

In 49A, mention ur PAN number to & tick appropriately, to make it clear that it's not for issue of new PAN.

apply for a duplicate pan card along with app.docs.

Mr. u.s. sharma forn 49a is for new pan card/ no. not for duplicate pan card . Fill form for duplicate pan card form can be downloded from the site of nsdl.
