by filling form 49B for correction in PAN.
fresh supporting documents for updation is required.
yes return of old pan card to department is also complusory
you should proceed though original PAN process trough authosied PAN centres only and no online system is available .

You have to apply for change requesting through such form. You have to submit your proof of address and identity proof along with proof to new name. You can apply it online and the fee will be pay through online by internet banking or credit card before going to apply for online please read the instruction to apply through online.
Not required to send old card to Income tax department.
