24 July 2012 Hey I have lost the PAN CARD of mine so for new pan card i have to file up form 49A again........my question is whether form for new application and form for lost pancard/ change in name is one and the same kya?????

25 July 2012 A duplicate PAN card request can be made by submitting 'Request for New PAN Card or/and Changes or Correction in PAN data' Form. This form is not the same as Form 49A.

25 July 2012 request for new pan card /correction in pan data and then you give supporting documant with them . not require fresh application

05 August 2012 For change in name you have to taken a new PAN Card so while making amendment in the duplicate PAN Card you apply online as suggested by other experts
