What is the procedure for cancellation of PAN?
and which type of form is to be filled up and submitted?

request for change in PAN or correction in PAN form 49A is to be used .just tick the option at bottom i.e.surrender of PANs and attach copy of PAN card to be surrendered alongwith ID and residence proof.

However, in form 49A there is no any space at bottom for options like surrender of PAN
